Registered Nurse (RN) | Case Manager Location: Burbank, CA Agency: Revolution Healthcare Services Pay: $2,282 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/16/2026 About the Position Job Opportunity – Medical Social Worker – California Start Date: ASAP Location: Southern California Compensation: Details provided to qualified applicants Schedule: Day shift with caseload up to 25 patients; typically one full weekend per 4-week rotation, subject to department needs Highlights: Acute care setting focusing on care coordination and discharge planning Patient population includes adults and geriatrics with diverse medical and social needs Epic charting system utilized for documentation Business attire or scrubs accepted 40-hour comprehensive orientation provided Collaborative interdisciplinary team environment Requirements: Active California state license (pending license accepted, but must be active upon start date) MSW or LCSW credential required Minimum 2 years of acute medical social work experience performing both social work and discharge planning functions Previous travel experience required; first-time travelers must be pre-screened Epic charting system experience required Knowledge of substance abuse treatment options, homelessness resources, mental health crisis intervention, advance directives, POLST, and protective services processes Demonstrated ability to organize and facilitate family meetings and participate in treatment plan development Maximum 7 days requested time off during assignment Commitment to work 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) Why It's a Great Opportunity: This position offers the chance to make a meaningful impact on vulnerable patient populations in an acute care environment where your expertise in both social work and discharge planning will be valued daily.
