Job DescriptionPosition Located in Rosemead, CAWe are seeking a Social Worker Assistant to join our team! You will provide emotional and administrative support to clients in either individual or group settings.Responsibilities:Assess nature of clients' situations by interviewing and reviewing personal historyDevelop and execute individual treatment plansDetermine appropriate milestones to gauge client progressCommunicate client updates to all relevant partiesFacilitate referrals to other healthcare professionals and programsMaintain accurate client documentationQualifications:Graduate must have Bachelor degree in Psychology, Sociology or Social Science.Prefer applicants who bilingual in Chinese, Cantonese, or Vietnamese. No Previous experience in social work, counseling, and interviewing skills needed, will trainCompassionate and caring demeanorAbility to build rapport with clientsStrong work ethic and punctual and willing to learn.Excellent written and verbal communication skillsCompany DescriptionA privately run Adult Day Health Care Service (Community Based Adult Services Program) in San Gabriel Valley that services such as skilled nursing, social service, physical therapy, occupational therapy, nutrition, and activity program to qualified participates who are 18 years and over. We are licensed by the Department of Public Health and CA Department of Aging. The center is open Monday through Friday from 8am to 3pm.
